Sorry to hear this; persistent reflux secondary to a hiatus hernia leading to longterm PPI use is a common and frustrating scenario. Difficult to give a personalised answer without knowing your whole situation. In short, patients with refractory reflux due to a hiatus hernia benefit from a specialist opinion to discuss the next options (pH and manometry testing, food diary, lifestyle measures, surgical and endoscopic options)
This is a very well done post. I think we agree that in certain circumstances, a risk/benefit analysis favors long term or even indefinite use. Without question, these proton pump inhibitor drugs are wildly overprescribed or continued on longer than is necessary. I have been guilty of this myself!
I have been taking PPI's for at least 8 years due to GERD. None of my doctors have ever questioned the continued use so I stopped taking it 10 days ago. I have less symptoms (burping, regurgitation) than with the PPI. I am determined to stay off as many prescription meds as I can. By the way, I am 80 years. old.
I take coated asprin 75mg most days. I get this acid reflux but only whilst walking. This is recent occurrence. Never had any issues before. I have been prescribed Lanzaparole which makes my stomach bloated. I also take bisoprol ramipril and a statin. Could these give me stomach ulcers? I also take a bergamot supplement 500mg twice a day.

You definitely need specialist input. Sounds like you are not responding to first line therapy (PPI) and need further options
Stricture occured when off PPI. Concerned about ppi long-term, but have tried all other eoe drugs on market. Under a specialist here in UK
