Mederteranian diet...... Do you actually know what this is??? In brief... Some scientists, visited a Greek island, during 'lent', or nistia, the time before Easter, when religious people fast(limit food) And observed the locals.... After a limited 4-6 week period, the Mederteranian diet was discovered!! Ta ta... Go look at your food pyramid...
Such a clear, actionable post and I love that you frame MASLD as a treatable metabolic signal rather than a moral failing.
From a clinician–scientist lens, the “big rocks” you emphasize are exactly what moves liver fat and downstream fibrosis risk in the real world:
1. Cutting sugary drinks / added fructose is often the highest-yield first step because it meaningfully reduces de novo lipogenesis without requiring perfection everywhere else.
2. A Mediterranean-style pattern + protein-forward meals is a practical way to improve insulin resistance and reduce hepatic fat while preserving lean mass.
3. The combo of Zone 2 aerobic work + resistance training is underappreciated for MASLD: it improves hepatic insulin sensitivity even before major weight change.
4. And thank you for including sleep, it’s an upstream lever for appetite regulation, insulin sensitivity, and adherence.
One nuance I’d add for our longevity-focused readers: fatty liver can be “silent” (normal ALT/AST), so it’s worth pairing lifestyle changes with risk stratification (FIB-4, and FibroScan when indicated) and tracking objective markers over time (A1c/fasting insulin, triglycerides, apoB, BP, waist circumference), not just LFTs.
We can also opt for black coffee to help reduce fatty liver.
Very useful if horrifying.
I would say treatable rather than horrifying 😀
Excellent article. Is ultrasound the best diagnostic method?
MRI

Reversed mine be eating a healthy diet including lean meats and exercise. No supplements, no nothing..

I had mild fatty liver. I lost 40 lbs on a plant-based diet and reversed it. My Fibrospect score is F0-F1.
What about flavored (pomegranate) sparkling water (Sanpelligrino)? It's my replacement for wine.
Silymarin (400 mg) should be part of any MASLD/MASH treatment plan. Zero harm and only upside.
